@Matt F You were correct. Sorry I didn't come back earlier and reply, but it took me a minute to realize you only get one shot when populating the LOOKUP map. After the first try, you must restart Websphere if you change it to a different value.

Yes I have tried doing a table domain and referencing it in the new location field that I put on the screen. It's so odd to me because the requested by and GL account fields I put on the screen work just fine. I'm going to keep toying with it and seeing what I can some up with. Thanks so much for your help!

Have you created a table domain for this on your custom attribute? How else are you linking the data between your custom field and the existing object/table (ie. Locations) to your new lookup? You mentioned "finished the DB config" - just confirming if that step was included or not. See below:
